Connie Bonebrake works to advance Humana’s Health Equity and Social Impact strategy as a market-based Population Health Lead focused on NC and Virginia. Connie has a bachelor’s degree in social work from East Carolina University and a master’s in social work with a concentration in aging from UNC-Chapel Hill. Connie has held various administrative roles including nursing home administrator, Vice-President of Post-Acute Care Services and Chief Patient Experience Office at a large integrated health system based in Charlotte. Immediately, prior to coming to Humana, she was Executive Director of a non-profit physician practice serving patients living with dementia or Parkinson’s Disease and their families. Throughout her career Connie has advocated for seniors and caregivers, championed diversity and inclusion and served as a formal and informal mentor for young professionals particularly women. Connie is “Mom” to three adult children and “Nonnie” to four grandchildren.
